ITEMS NEEDED WHEN SCHEDULING A STRESS TEST
When a stress test is scheduled at the Cardiovascular Group of Syracuse the
following items are requested:
1. A signed order from the
referring provider designating exactly what type of study is to be performed.
2. A recent EKG, if available.
3. The patient’s last office note with medication list,
symptoms and medical history.
The Cardiovascular Group requests this information at the time the
appointment is scheduled. This will give us the proper amount of time to go over
the patient information and order proper doses of medications necessary to
perform the test.
We can not guess what type of stress test is requested. If the order is
marked stress test, a regular stress test will be performed even though your
providers may have wanted a more appropriate test. Having your office records
well before the test will help our physicians avoid inappropriate testing.
ITEMS NEEDED WHEN SCHEDULING A CONSULT
When a Consultation is requested at the Cardiovascular Group of Syracuse, the
following items are requested:
1. The patient’s last office note with medication list, symptoms and medical
history.
2. Most recent EKG.
3. Other pertinent information (echocardiogram, Holter monitor, lab work and
other relevant test results).
Please fax or mail these records as soon as possible. We hope to minimize phone
calls and disruption of your office flow.
